In accordance with Texas Health and Safety Code §181.105 and House Bill 4224, the following information is provided to help consumers understand their rights, how to access health care records, and available complaint resources.

Requesting Your Health Care Records

You have the right to request a copy of your health care records.

To request records from Resilient Hearts Counseling and Consulting, please contact the practice directly at (210) 549-8475. A records request form will be provided for you to complete.

Records will be provided in accordance with applicable Texas and federal law and privacy requirements. If you have questions about requesting or accessing your records, please contact the practice for assistance.

Filing a Consumer Complaint

Consumers may file a complaint concerning patient privacy, access to health care records, or other matters covered by the Texas Medical Records Privacy Act through the Texas Office of the Attorney General.

Filing a Complaint About a Licensed Professional

Complaints regarding the professional conduct of a Texas licensed professional counselor may be filed with the Texas Behavioral Health Executive Council.
